- CHECKLIST OF REQUIREMENTS FOR CASE DOCUMENTATION
Prior to the collection of biological samples, the following are needed:
a. Completion of Form 001. Give details of purpose for DNA analysis.
b. Submission of one recent 2” x 2” ID photo, white background (taken in the last week) signed at the back to be included in Form 002.
c. Submission of a photocopy of the child’s birth certificate accompanied by the original (for verification) and the couple’s marriage certificate (when applicable).
d. Signing of the appropriate Agreement by the requesting party and his/her witness.
